Participant Waiver, Parent/Guardian Consent & Media Authorization

1. Participant and Event

This waiver applies to the player identified in the registration form and the 2027 FourPar3z Junior Championship scheduled for June 14–15, 2027 at Champions Golf Course in Columbus, Ohio. “Released Parties” means FourPar3z LLC, the host golf course, event sponsors and each of their respective owners, officers, employees, agents and volunteers.

2. Acknowledgment and Assumption of Risk

I understand that participation in golf and attendance at a golf facility involve inherent and other risks, including being struck by golf balls or clubs, slips and falls, uneven or wet terrain, weather exposure, lightning, heat-related illness, interaction with carts or other equipment, travel within the facility and the acts or omissions of participants or other persons. I knowingly and voluntarily accept these risks for myself or the minor player identified in the registration.

3. Health, Fitness and Tournament Rules

I represent that the player is physically able to participate and will follow tournament rules, course policies, safety instructions and weather-suspension procedures. I will disclose relevant medical or accessibility information reasonably necessary for safe tournament administration and will not permit participation when the player is medically unable to compete safely.

No on-course coaching or communication: From the time the player begins a tournament round until the scorecard has been returned, parents, coaches and spectators must not talk to, signal, text, call or otherwise communicate with the player. Communication is permitted only for an emergency or when specifically authorized by a tournament official. I understand that a violation may result in a penalty or disqualification.

4. Release and Covenant Not to Sue

To the fullest extent permitted by Ohio law, I release and agree not to sue the Released Parties for claims arising from ordinary negligence connected with participation in or attendance at the event, including personal injury, illness, death or property loss. This release does not apply to conduct that cannot legally be released, including gross negligence, reckless conduct or intentional misconduct.

5. Emergency Medical Authorization

If I cannot be reached during an emergency, I authorize tournament personnel and the host facility to obtain reasonable emergency medical assistance for the player. I understand that I remain responsible for medical expenses and that FourPar3z does not provide medical insurance for participants unless expressly stated in writing.

6. Parent/Guardian Authority

If the player is under age 18, I certify that I am the player’s parent or legal guardian and have authority to provide this consent. If the player is age 18, the player may accept and electronically sign this waiver directly.

7. Tournament Photography, Video and Media Authorization

I understand that the FourPar3z Junior Championship will be photographed and filmed. Tournament photographs, video recordings, audio recordings and interviews will be used for event coverage and may be published on the FourPar3z website, official YouTube channel, social-media accounts, email communications, printed materials and future tournament promotions.

I authorize FourPar3z LLC and its authorized photographers and videographers to capture, edit, reproduce and publish the player’s name, image, likeness, voice, interview and competition footage for these purposes without compensation. I understand that published online material may be viewed, shared or downloaded by others.

This authorization does not permit FourPar3z to sell the player’s personal information or imply the player’s endorsement of an unrelated third-party product. A parent or guardian with a specific safety or privacy concern should contact info@fourpar3z.com before registering.

8. Electronic Acceptance

By entering the waiver signer’s full legal name and selecting the waiver-acceptance checkbox on the registration form, I intend to sign this waiver and media authorization electronically. I consent to the use of electronic records and acknowledge that FourPar3z will retain the signer’s name, waiver version and acceptance timestamp with the registration record.

9. Governing Law and Severability

This waiver is governed by Ohio law. If any provision is found unenforceable, the remaining provisions will continue to the fullest extent permitted by law.
